Terms of Service
1. Services Provided Eden Integrated Health & Wellness offers Direct Primary Care (DPC), urgent care visits, and chronic condition management. We reserve the right to withdraw, cancel, or modify services at any time.
2. Financial Transactions All payments are due at the time of service unless otherwise agreed upon. We reserve the right to update our pricing structure.
3. Customer Expectations While we strive to provide accurate information and uninterrupted access to our services, we are not liable for errors in information, technical issues, or website downtime.
4. Copyright and Intellectual Property All content, including text, graphics, and logos, is the property of Eden Integrated Health & Wellness. Users may not reproduce, distribute, or use content for commercial purposes without prior written permission.
5. User Behavior Users are expected to use our services lawfully and respectfully. Unlawful behavior, hate speech, bullying, spam, or unauthorized promotions are strictly prohibited. 6. Account Management We reserve the right to suspend or disable user accounts in cases of misuse or violation of these terms.
7. Liability Eden Integrated Health & Wellness is not liable for damages arising from the use of our services, beyond what is required by applicable law.
8. Agreement By using our services, you agree to these Terms of Service. These terms may be updated periodically, and continued use constitutes acceptance of the revised terms.
Refund Policy –
Eden Integrated Health & Wellness At Eden Integrated Health & Wellness, we operate under a Direct Primary Care (DPC) model. Membership fees support access to ongoing primary care services and are distinct from traditional health insurance. The following refund policy outlines your rights and responsibilities as a member. Voluntary Termination Patients may cancel their membership at any time with at least 30 days’ written notice. Upon termination, any prepaid fees for unused time will be refunded on a pro-rata basis. Automatic Renewal Membership agreements may automatically renew on a monthly or annual basis unless the patient provides notice of non-renewal. Refunds will be issued for any prepaid amounts beyond the termination date. Practice-Initiated Termination Eden Integrated Health & Wellness may terminate a patient’s membership with 30 days’ notice for reasons including: • Failure to pay membership dues • Abusive behavior or posing danger to staff or patients • Fraudulent activity • Discontinuation of the DPC program No-Refund Services Membership fees cover the primary care services listed in the membership agreement. Additional services, such as labs, imaging, or specialty care, may require separate non-refundable fees. Administrative or Enrollment Fees Any one-time enrollment or administrative fees are non-refundable. Re-enrollment Patients who cancel and later wish to rejoin may be subject to a new enrollment fee and/or waiting period. Compliance with State Law In accordance with Florida law, DPC agreements must allow patient termination with at least 30 days’ written notice. If Eden Integrated Health & Wellness ceases to provide services, prepaid monthly fees will be refunded in full.
